Facelift/Rhytidectomy

Signs of aging in the face and neck, such as loose skin and jowls, can hide our natural aesthetic contours, giving an aged appearance. A Facelift, also known as Rhytidectomy, repositions and reshapes loose or sagging skin and underlying tissue to restore a more youthful appearance that enhances one's own natural features, without looking pulled or unnatural.

Dr. Burke understands no two patients are alike, and customizes your procedure to your specific needs. Your individual consultation with Dr. Burke will reveal which combination of procedures will be necessary to achieve your optimal results. It is recommended to bring a photograph of yourself from 10-15 years past to assist with obtaining results that reflect your individual features.

Surgical rejuvenation may address the mid-face, the cheeks and jowls, and the neck through incisions inconspicuously placed around and behind the ears.

Some patients will benefit from non-surgical treatment, such as BOTOX® Cosmetic, fat grafting, or facial fillers. These non-surgical options alone may refresh your appearance or delay the signs of aging, or may complement a facelift and refine natural-looking outcomes or maintain your more youthful appearance.

Cheeklift/Midface Lift

Large bags under the eyes extending down to the cheek or giving the appearance of a "double bag" are known as malar bags. Along with loss of loss of midface volume and fullness in the cheeks, malar bags can result in an aged, tired appearance. Dr. Burke addresses these areas of the face with a Cheek Lift, also known as a midface lift, which repositions and redrapes sagging tissue, resulting in smooth, rounded and more youthful midface contour.

Dr. Burke often performs a Cheek Lift in combination with other procedures, such as eyelid surgery or blepharoplasty, to achieve your optimal outcome.

While natural aging will continue after your midface lift, skin care and non-surgical rejuvenation of the face, such as facial fillers, can be used to maintain a youthful appearance after surgery.

Necklift

Loose and dropping neck skin can add years to your appearance. Excess fatty tissue may result in the appearance of a double chin and can give a heavy appearance to the face. Surgical treatment of the signs of aging in the neck, also known as a Necklift, reposition and reshape loose or sagging skin and muscle, and may remove fat deposits in the neck and beneath the chin. Dr. Burke focuses on a natural-appearing result – a smooth, contoured neck, with visibly defined angles of the jaw and a youthful profile.

Dr. Burke customizes each procedure to your specific needs and may perform a combination of techniques to achieve your desired outcome. In patients with good skin elasticity and no sagging, liposuction alone can achieve the desired result by treating excess fat deposits beneath the chin. Liposuction, in combination with a Necklift, is used to address both excess fat and loose, wrinkled skin.

Rejuvenation of the neck is performed through inconspicuous incisions behind the ear and additionally one may be hidden in the crease under the chin.

Nonsurgical Facial Rejuvenation

Facial Fillers/ Juvederm®, Restylane®, Prevelle®, Radiesse
As we age, our face loses volume, accentuating lines in the cheeks, under the eyes, around the lips and prejowl areas. Facial fillers work instantly to smooth wrinkles, fill deeper lines, and restore volume, giving a more youthful appearance to the face.

Gel fillers, such as Juvederm®, Restylane®, Prevelle®, and Perlane®, are formulated from hyaluronic acid, a substance naturally found in the skin. These products are used to restore fullness and volume. Results are seem immediately at the time of treatment and may last for up to one year.

Radiesse is made from calcium hydroxyapatite, a substance found in bone, and is used for deeper lines and volume correction in the face. The calcium particles provide a platform for collagen production, with longer-lasting results of up to 18 months or more. Radiesse may be used in conjunction with gel fillers to create a natural result in addressing aging facial features. Unlike gel fillers, Radiesse is not intended for placement around the lips.

Facial fillers may be used alone, in combination with one another, or in combination with surgical rejuvenation. Dr. Burke customizes your treatment options specific to your individual needs.

Botox® Cosmetic /Dysport®
Continuous movement of the skin on our forehead and around the eyes can cause lines and wrinkles that remain visibly etched when the face is at rest. Botulinum Toxin type A, the active protein in both Botox® Cosmetic and Dysport®, relaxes the muscles in these areas, softening frown lines, forehead furrows and crow’s feet around the eyes. The result is a natural appearance, and a smooth, more youthful look to your face.

Dr. Burke also uses Botox® and Dysport® to treat excessive axillary (underarm) sweating. This is done through a series of small injections in the hair-bearing areas of the underarm region.

You may begin to notice results after 48-72 hours, and full results will be seen after one week.

Results generally last 3-4 months. In order to maintain your results, injection will need to be repeated as muscle contraction and expression recur.

Nose Surgery/Rhinoplasty

The nose provides balance and harmony as the central feature of the face. Surgical shaping of the nose, also known as Rhinoplasty, improves the aesthetic features of the nose, including shape and length of the bridge, tip and nostrils, and may also treat septal deviation or other problems that impair breathing through the nose.

Dr. Burke uses refined surgical techniques to achieve subtle, natural outcomes that result in an attractive balance of your facial features and avoid an "overdone" appearance.

What to expect
Generally outpatient, rhinoplasty is performed under general anesthesia and may require an overnight stay when combined with other procedures.

Recovery depends on the extent of your nose surgery, and any accompanying procedures. You may engage in light activity the day after your surgery; a return to more normal activity may take 1-2 weeks. The appearance of your nose will gradually refine over the following 4-6 weeks, although it may take a 6-12 months for your final shape.

Results from surgical shaping of the nose are generally permanent, however the cartilage and soft tissue of your nose may move or change over time.

Ear Surgery/Otoplasty

Ears that project significantly can offset overall facial harmony and may results in a source of embarrassment or hinder self-confidence in adults and children. Surgical revision of the ear, also known as Otoplasy, improves the angle and appearance of protruding ears.

Dr. Burke uses up-to-date techniques that hide the incisions behind or inside the ear to create a natural, balanced appearance.

What to expect
Generally outpatient, otoplasty is most performed under either general anesthesia or local anesthesia with sedation.

Recovery depends on the extent of your ear surgery, and any accompanying procedures. You may engage in light activity the day after your surgery; a return to more normal activity may take 1-2 weeks. Your appearance will gradually refine over the following 4 -6 weeks.

Results from otoplasty are generally permanent.

Chin Surgery/Genioplasty

A well-defined, proportioned chin provides balance to the face and profile appearance. A small or deficient chin, or conversely an oversized chin, offsets facial harmony and greatly influences profile appearance. Chin augmentation, or genioplasty, surgically improves the shape of the chin by adjusting the chin bone surgically.

Dr. Burke has advanced fellowship training in craniofacial surgery, a subspecialty in plastic surgery that focuses specifically on the bones of the face, and recognizes the importance of using or moving a patient's own natural tissue to avoid complications that may arise with the use of artificial implants.

What to expect
A genioplasty may be performed in conjunction with other surgeries, such as rhinoplasty, to achieve the ideal facial proportions and balanced profile.

The surgery is performed through incisions hidden inside the mouth.

Generally outpatient, chin surgery is performed under general anesthesia and may require an overnight stay when combined with other procedures.

You may engage in light activity the day after your surgery. A return to more normal activity may take up to 2 weeks. Your appearance will gradually refine over the following 6 weeks.

The results following chin surgery are generally permanent.

Skin Cancer Repair after Mohs' Surgery

Treating skin cancer is often a two phase approach: removing the cancerous tissue, then repairing the resulting defect in the skin with a vision for the best possible aesthetic outcome.

Dr. Burke uses specialized techniques to restore facial appearance following the removal of skin cancer lesions and Mohs' surgery, which includes replacement of the removed skin with grafts or flaps of adjacent tissue and the use of suture techniques to minimize visible scarring.

What to expect
Generally outpatient, skin cancer repair is commonly performed under local anesthesia possibly with mild sedation.

You may engage in light activity the day after your surgery. A return to more normal activity may take 1-2 weeks. Your appearance will gradually refine over the following 6-8 weeks as the scars heal; secondary procedures including dermabrasion or laser treatments may be recommended to further improve your results.

Brow Lift: Endoscopic/Temporal Lift

Frown lines, forehead wrinkles, and sagging eyebrows can give the impression of sadness, anger or fatigue. A Brow Lift improves signs of aging by smoothing frown lines, wrinkles and expression lines on your forehead and may reposition sagging eyebrows that cause hooding of the upper eyelids.

Dr. Burke will customizes each procedure, or combination of procedures, to your unique features and desired outcome. She utilizes contemporary techniques that are demonstrated safe and effective. Minimal scarring through endoscopic instrumentation and shorter, temporal incisions hidden behind the hairline restore a naturally younger, alert appearance.

For patients with only drooping of the lateral, or outer ends of the eyebrows, a Temporal Lift elevates the tail of the brow and temple area to a natural position, through small incisions. This minor adjustment can dramatically improve the saddened appearance associated with brows that arch downward.

For patients with extensive sagging of the upper eyelids Dr. Burke may recommend Eyelid Surgery, or Blepharoplasty at the time of Brow Lift.

Results are often long-term, however natural aging cannot be prevented. The muscles that create the frown lines and furrows are generally removed therefore the improvement in the appearance of frown lines can be long lasting.

Upper Eyelid/Upper Blepharoplasty

The eyes are the most studied and most expressive feature on your face. Hooding of the upper eyelids or drooping upper eyelids can impart a tired look to your overall appearance even though you may feel rested, or make you appear aged even though you may be a young adult.

Dr. Burke uses skills acquired during her specialized training in eyelid surgery that focus on natural-appearing refinement of the upper eyelid. By rehaping the upper eyelid, removal of excess skin, and added specialized techniques that include removal or repositioning of fat pads, Dr. Burke can correct the upper eyelid fold and restore a more youthful, naturally rested and happy appearance.

Generally an outpatient procedure, upper eyelid blepharoplasty is most commonly performed under local anesthesia with sedation.

Lower Eyelid/Lower Blepharoplasty

Lower eyelid puffiness, bags, and excess wrinkles or hollows beneath the eyes can create a fatigued and aged look to your overall appearance. Cosmetic surgery of the lower eyelid, or lower blepharoplasty results in a smooth, youthful appearance to the lower eyelid, and lower eyelid/cheek junction also called a tear trough.

Dr. Burke uses techniques acquired during her specialized training in eyelid surgery that focus on natural-appearing refinement of the lower eyelid. Fat causing puffiness and bags in the lower eyelid region may be reduced or repositioned. Where there exist hollows, fat may be grafted to create a smooth, firm lower eyelid. Where necessary, canthopexy or canthoplasty a reposition or tightening of the lower tendons, may be recommended to create or restore a natural eyelid shape and support the lower eyelid. Excessive wrinkling or bagging of lower eyelid skin may be corrected through a reduction of the skin. Additionally fat distribution may improve depressions within the inner corner of the eyes or the tear trough. A cheek lift or midface lift is an option for patients with malar bags, or sagging of the skin on to the cheek.

Droopy Eyelids/Ptosis Correction

Droopy upper eyelids that rest too low on your eye (also known as eyelid ptosis) can impair your peripheral vision in addition to having a tired appearance. The procedure to correct this, known as Ptosis Correction, repositions the upper eyelid by repositioning the eyelid muscles and reduces excess, obstructing skin to lift the position of the eyelid margin and create a smooth, natural contour to the upper eyelid.

Dr. Burke uses the most up-to-date techniques learned during her specialized training in eyelid surgery to focus on natural correction of drooping eyelids and eyelid ptosis.

Generally outpatient, correction of droopy eyelids is most commonly performed under general or local anesthesia with sedation.

Repair of Prior Eyelid Surgery

Surgical correction can restore a natural shape, position, and function of the eyelid where prior eyelid surgery has produced unsatisfactory results such as:

  • difficulty closing
  • excess sclera show (where the white inside the lower eyelid is revealed)
  • unnatural rounded or tight eyelids

Dr. Burke uses the detailed techniques learned during her specialized training in eyelid surgery to perform Eyelid Repair. In some cases correction of prior eyelid surgery may include placement of internal eyelid spacers, as well as lateral canthopexy or canthoplasty (repositioning of tendons) to elevate the lower eyelid back to a more natural position.

Skin Cancer

Treatment to remove skin cancer near or on the eyelid requires specialized techniques to maintain natural function and appearance of the eyelids.

Dr. Burke uses very detailed, specialized techniques to remove and/or repair skin cancer excisions, which may include skin grafts and flaps. The reconstruction is often performed the same day after Mohs’ skin cancer removal.
